The clean-up rules are a bit more complex than that, but generally it is based on density limits that we define per entity type. Some entities we want to stick around in larger quantities for longer as they provide gameplay or loot, while other entities are cleaned up more aggressively such as hospital gowns. We can define different limits for streamed in and streamed out entities, as we want to avoid as much noticeable clean-up happening in front of the player's eyes, without deteriorating server performance.
